MDM Forms SLA Reminder

Dear All,
It is our commitment to deliver quality and timely master data maintenance, thus, to ensure that established processes and timeframes are met -- we would like to remind everyone of the MDM Forms SLA (Service Level Agreement). Please refer to the table below:
Please also be reminded of the following :
Always make a copy of the form from MDM Forms folder to ensure file being used is updated and save it in your own Google Drive. Any copied forms saved in MDM folder will be deleted automatically.
Do not alter any row or column in the form such as adding and deleting, as this will affect the form script
To add requestor, email us using the format below :

Department :
Requestor Name & Email Address :
Immediate Superior Name & Email Address :
Division Head Name & Email Address :
We encourage every new hire or whoever feels the need to be refreshed of the MDM Forms process to request a refresher session from MDM
